New Patient Consultation: Up to 35% Off NOW $495
- For new patients, we are collecting $495.00 via credit card when booking the 1st appointment.
- If you need to reschedule, the credit will be on hold until you come into the office.
- Same day cancellations will result in a $100.00 NON REFUNDABLE
- Follow-Up Visits: $395
- Phone Consultation: $395
- TRH Stimulation Test: $749
- Heavy Metal Testing: $349
Payment Policy
Payment is due, in full, at the time service. Payments may be made with any combination of the following:
- Cash
- Checks
- Mastercard
- Visa
- American Express
- Discover Card
In the event we receive a returned check, due to insufficient funds or closed accounts, you will be charged $50. If your account is turned over to a third party for collection, there will be a charge of at least 50% of your total balance to cover attorney fees and other collection costs.
If you have insurance, we will provide you with the standard form(s) typically needed for filing a claim when you checkout.
*Dr. Salerno is an out of network practitioner. This means you will be responsible for all consultation, follow up and IV treatment costs. However, you can be reimbursed from your insurance for visits and treatments, granted your deductible has been met
Cancellation Policy
48-hour notice is required to cancel an established patient appointment.
Cancellations for Monday appointments must be made by 12 noon on Friday to avoid being charged.
Our missed appointment policy is strictly enforced and is intended to prevent ALL patients from having to pay higher fees due to the irresponsibility of a few.
*The fee for a broken appointment will range from $100 up to $250 or 50% of the cost of the appointment. Same day cancellations will result in a $100.00 NON REFUNDABLE Cancellation fee.
